Hennepin Health is an award-winning managed care program that serves Hennepin County residents ages 0 to 64 who are eligible for Medical Assistance (Medicaid) or MinnesotaCare. Launched in January 2012, the program is a collaboration among Hennepin Health, Hennepin County Medical Center (HCMC), NorthPoint Health and Wellness Center, and the Human Services and Public Health Department of Hennepin County.
Hennepin Health takes an integrative approach to health care by considering a member’s medical, behavioral health and social service needs. Hennepin Health-PMAP and Hennepin Health-MNCare members receive care from a multidisciplinary care coordination team that consists of doctors, nurses, pharmacists, social workers and community health workers. Other innovative features of the program include a common electronic health record, along with a tiered care system that is based upon a member’s identified needs.
