Hennepin Health provides health care coverage to Hennepin County residents who are enrolled in a Minnesota Health Care Program. To see if you are eligible to enroll as a member in one of the plans, review the information below.

Hennepin Health-PMAP & Hennepin Health-MNCare

Hennepin Health-PMAP and Hennepin Health-MNCare are managed care programs that offer medical, behavioral health and social services for those who are: 

  • A resident of Hennepin County
  • Between the ages of 0 and 64
  • Eligible for Medical Assistance (Medicaid) or MinnesotaCare

Hennepin Health-SNBC

Hennepin Health-SNBC is a Special Needs BasicCare plan for those living with disabilities who are:

  • A resident of Hennepin County
  • Between the ages of 18 and 64
  • Eligible for Medical Assistance (Medicaid)
  • Certified disabled (by a State Medical Review Team or through Social Security Disability Insurance)
