Continuity of care

In accordance with the Minnesota Patient Protection Act, you have the right to know how Hennepin Health provides for continuity of care in the event a network contract is terminated.

When a network clinic is no longer part of the Hennepin Health network, members will be informed of the closing within 30 days or as soon as possible. Members must select a new network clinic from the list of remaining clinics, which can be found in the provider directory.

Members with special medical needs can obtain an out-of-plan referral to continue care at their current clinic. Some of these special medical needs include:

  • Members who require ongoing medical attention such as cancer therapy, mental health or chemical health therapy, or women in their second trimester of pregnancy
  • Members with cultural or language barriers

To request continuation of care at your current clinic, you must send a written statement to Hennepin Health asking for approval to do so. Requests are reviewed on a case-by-case basis.
