Special Needs BasicCare (SNBC) member transition

Please be aware of an important change starting October 1, 2025

If you were seeing a HealthPartners or Park Nicollet provider prior to joining Hennepin Health on April 1, 2025, you can continue to see your HealthPartners or Park Nicollet provider until September 30, 2025.

Starting October 1, 2025, your Hennepin Health insurance will not cover care and services within the HealthPartners/Park Nicollet provider network. (This is based on a decision by HealthPartners/Park Nicollet to not accept Hennepin Health insurance.)

For help finding a new primary care and/or specialty care provider(s), contact your SNBC care coordinator or call Hennepin Health Member Services at 612-596-1036 (TTY 711).
