We are Minnesota’s only county-owned, state-certified health maintenance organization. As a managed care organization, our outstanding, collaborative team takes a holistic approach to health care by integrating traditional medical services, behavioral health services, and other county and community services that address the social influencers of health.
We serve Hennepin County adults, families and children enrolled in Minnesota Health Care Programs. We offer three health plan products:
Our network centers around our accountable health model and includes approximately 17,000 providers at 1,700 locations. Our members receive care from a multidisciplinary care coordination team that consists of doctors, nurses, pharmacists, social workers and community health workers.

We believe our organization is stronger when we listen to our members. In addition to collecting feedback daily through Member Services, we have two committees that meet regularly to share their ideas and experiences.
