Background

Hennepin Health has made a change to the drug formulary. This change applies to Hennepin Health-PMAP, Hennepin Health-MinnesotaCare and Hennepin Health-SNBC members. Members who are directly impacted will receive notification of the changes. The updated full drug formulary is available on the website. A printed copy can be obtained by calling Provider Services at 612-596-1036 (press 2).

Effective Feb. 1, 2024, this medication and utilization management (UM) requirements of prior authorizations (PA) was added:

• Ferrous sulfate 220 mg/5ml (44mg/5ml elemental fe), PA required for age 8 or older
