Providers can now use the Hennepin Health Nursing Facility Communication Submission eForm to attach and submit completed DHS Nursing Facility Communication Forms to Hennepin Health electronically. Providers will save time by easily uploading and submitting the document on the eForm instead of faxing the form.
The Hennepin Health submission eForm and a fillable blank copy of the Nursing Facility Communication (DHS 4461) form are available on Hennepin Health’s website: Forms / formulary | Hennepin Health, below “Service authorizations.”
