Advance directive planning encourages discussions and decisions between providers, members, and family members regarding the member treatment preferences for a chronic illness/condition and/or end-of-life care. The advance directive assists family members or caregivers when the member can no longer participate in health care decisions. Hennepin Health conducts an audit each year in accordance with regulatory standards to evaluate advance directive rates among our members. In 2024, Hennepin Health members who were included in the PMAP and MinnesotaCare sample for this audit had an advance directive planning discussion rate of 19.95%, a 7.06% increase compared to 2023. However, the rate of an advance directive being present in electronic health records (EHR) in 2024 was 4.87%, a decrease of 1.45% when compared to 2023. The rate of advance directives in the member’s EHR or conversations about advance directive planning with providers continues to be low for this population. Providers should continue to discuss advance care planning with patients.
