Register as a non-contracted provider

Non-contracted providers do not have signed agreements with us; they are considered out of network.

As a non-contracted provider, it is important to understand our provider registration, billing guidelines and claims processes to avoid delays in claims payment. 

Refer to our non-contracted billing guidelines found in the Provider Manual. 

Step 1: Register with the Minnesota Department of Human Services

Non-contracted providers are required to be enrolled with Minnesota Department of Human Services BEFORE registering with us.

To register with MN Department of Human Services, complete the Minnesota Health Care Programs (MHCP) provider enrollment process. You can do this:

  • Online at mn.gov/dhs/partners-and-providers/policies-procedures/minnesota-health-care-programs/provider 
  • By email to dhs.healthcare-providers@state.mn.us.
  • By calling 651-431-2700 (800-366-5411), TTY: 711 (800-627-3529). Hours are 8 a.m. – 4:15 p.m., Monday-Friday.

Step 2: Register with us

Registering is not a request to contract or credential with us. Allow up to 45 business days for us to process your registration.

1. Complete the following documents:

  • Non-network provider information form
  • W-9/ACH form 

2. Submit the documents requested in step 1 to hhnetworkmanagement@hennepin.us.
