Hennepin Health PMAP is a managed care program that ensures access to the care and services our members need. If you live in Hennepin County and are eligible for Medical Assistance (MA), Hennepin Health allows you to choose from a wide variety of services and providers within the Hennepin Health network.
PMAP is for adults without children, pregnant women and families with children.

You can apply for coverage online. Complete the application for Minnesota Health Care Programs at https://www.mnsure.org/. You may also call or visit the Hennepin County Human Service Centers.

If you qualify, you will be contacted by Hennepin County, DHS or your tribal agency to choose a health plan. You may be able to request Hennepin Health for your health coverage.
You do not have to pay a monthly fee (premium) for this plan. You may have to pay a small dollar amount (co-pay) when you see the doctor or get a prescription filled.
