Managing your diabetes

Diabetes is a chronic disease that can lead to disabling and even deadly complications if it is not controlled. Controlling diabetes means keeping blood sugar levels and blood pressure levels near normal. Monitoring your blood sugar levels and blood pressure daily can be challenging and overwhelming – but there are tools and resources that can help. Learn more about diabetes management and how your care team can support your overall health. 

10 steps to manage your diabetes

  1. Take your medicines as directed – they only work if you take them
  2. Check your blood pressure as directed – know what your number should be, know what to do if it is too high or low
  3. Do not smoke or chew tobacco – get help to quit
  4. Check your blood pressure often – write it down and show your doctor and know what your numbers should be
  5. Eat healthy foods – add more fresh fruits and vegetables, cut down on fats, salt and sugar and watch your portion sizes
  6. Be active 30 minutes a day most days – walk, use a stretch band, dance, garden, or any activity that you like that keeps you active
  7. Check your feet every day – look for sores, blisters, color changes or hot spots
  8. Get help when you feel down – tell your doctor if the feeling doesn’t go away
  9. Relax and enjoy life every day 
  10. At least once a year: 
    • See your doctor
    • Visit your diabetes educator
    • See your dentist
    • Get an eye exam

Create a diabetes action plan

For people with diabetes, action plans are important. So, what is a diabetes action plan? It’s a plan to help you manage the things that may be hard to fit into your daily routine, like testing your blood sugar and watching your weight. Creating a diabetes action plan can help you set goals, manage day-to-day tasks, and prepare for unexpected illness.

Diabetes can impact almost every part of your body

From your brain to your eyes and all the way to your feet, diabetes can have negative effects on different parts of your body. By monitoring your blood sugar, you can prevent or delay health issues that are connected to diabetes.

Diabetes and mental health

There is a close relationship between diabetes and mental health. Like diabetes, your thoughts, feelings, beliefs, and attitudes can affect how healthy your body is. Anxiety, depression, distress, and other mental health concerns can impact how you manage your diabetes and other daily tasks. Untreated mental health issues can make diabetes worse. Likewise, problems with diabetes can make mental health issues worse. Fortunately, if one gets better, the other tends to get better too.

Care coordination

Our care coordination approach supports members’ health and human service needs. A care coordinator can work with you to connect and engage with primary care and the health care system.

Rewards for diabetes care

Members (18 years or older) who have a diagnosis of diabetes can earn a $50 gift card for completing an eye exam and a $25 gift card for completing an A1C test.

Flavors of Health

Learn how to cook nutritious food that is good for you by participating in the Flavors of Health program. Through the program, Hennepin Health members who take a cooking class at NorthPoint Health & Wellness Center or at Hennepin Healthcare Redleaf Center for Family Healing will receive 8 weeks of free groceries delivered to their home.. To participate you must meet one of these criteria:

  • Be pregnant
  • Have diabetes
  • Have congestive heart failure

There is no cost to participate.

Get mental health care

Your member benefits include behavioral and mental health care. We have an open network, which means you can visit any behavioral or mental health provider. This includes therapy and medications.
